1. No proper Covid-19 screening for patients in the emergency room area. 2. Failure to enforced the wearing of face covering/N-95 masks in the emergency room. 3. Covid-19 positive patients and non-positive patients are kept in the same isolation room. 4. Negative pressure rooms, waiting room, and triage area do not have functioning hepa filter. 5.
