1. Not taking temperatures or screening employees properly for symptoms of COVID-19 and not maintaining records consistently for the screening protocols. 2. Not using proper disinfectant solution for cleaning high touch surfaces 3. Owners not wearing masks in restaurant and not enforcing face coverings be worn to bathroom or on the way out of restaurant. 4. Not reporting positive COVID cases to health department for contact tracing. 5. Not properly spacing the customers seated on patio ensuring 6ft social distance. 6. Not ensuring PPE is consistently being used or properly worn. 7. No signs posted at entrance informing customers not to enter if they were recently sick
