1. Nurses are not being informed when they are in contact with a COVID-19 positive patient, visitor or staff member, when working in a non-COVID area and not wearing proper personal protective equipment. 2. Nurses have not been fit tested on N95 respirators for more than a year. They are not fit tested for all models of a respirator. 3. The hospital does not have effective screening of patients, visitors and staff entering the hospital. 4. Sometimes, COVID-19 patients are placed in hallways and are not isolated in non-COVID-19 areas of the hospital.
