1. Forklifts in the facility are lacking a back stop to prevent lifted loads from falling onto the forklift operators. 2. Employees are required to refill electric forklift batteries with water. There is no eyewash and safety shower present in the area. 3. Forklifts lift heavy loads over the heads of production personnel. 4. The rivet machines are missing their front panels and have their finger detection guarding bypassed. 5. The facility does not have an energy control program. Specific procedures have not been developed for machines with multiple sources of power such as the rivet machines. 6. Three of four glue tables are lacking guarding around chains and sprockets. 7. The facility's four single hit presses are lack guarding. Foot switches that do not require constant pressure are utilized to actuate the presses. 8. Guardrails are missing in the area of the paint tank. 9. Debris on the floor near the paint area prevent egress in the event of an emergency. 10. The facility has not developed and implemented an emergency action plan. No emergency routes, rally points, or shelter in place areas have been established. 11.
