1. Employee are potentially exposed to COVID 19 in that the employer is not adhering to CDC guidelines as it relates to SARS, COVID-19 virus. Not practicing social distancing, not enforcing wearing of face covering. Location: Warehouse. 2. Employees are exposed to slip, trip and fall hazards in that the employer does not perform housekeeping efforts on the back dock area. Boxes and trash allowed to excessive accumulate in that area. Location: Back dock (back of the warehouse). 3. Employees are exposed to fall from height (4ft+) hazards in that the employer does provide safety chains across the dock doors (17 dock doors). Location: Loading/receiving dock. 4. Employees are exposed to struck-by hazards from Material Handling Equipment in that the employer does not provide training to employees on MHE (2 electric forklifts, 5 electric pallet jacks) prior to employee operation. Location: Warehouse. 5. Employees are exposed to fire hazards in that the employer allows for emergency exit doors to be blocked by pallets . Location: 2 doors on the dock.
