1. The employer is not complying with the Governor's COVID-19 mandate in regard to employees wearing face coverings. Employees and members of management do not wear face coverings in the back of house. 2. The employer is not complying with the Governor's COVID-19 mandate in regard to persons/patrons wearing face coverings while in the Employer's establishment. Patrons are not wearing face coverings in the store. 3. Employees are at risk of a trip hazard due to unsafe placement of pallets. 4. Fire exits are blocked exposing employees to a fire hazard.
