1.) A permit-required confined space entry program for employees required to enter confined spaces (i.e., trash compactors) has not been developed or implemented. 2.) Employees who are expected to use fire extinguishers to fight incipient fires have not received training. 3.) Fire extinguishers have not received monthly or annual inspection. 4.) A lock out/tag out program with machine specific procedures has not been developed or implemented. 5.) The Hazard Communication program and safety data sheets (SDS) have not been updated to comply with the Globally Harmonized System (GHS). 6.) Employees working with corrosive chemicals do not have access to eyewash stations. 7.) No one onsite has been adequately trained to render first aid. 8.) First aid kits are not readily available. 9.) Employees have not been trained according to manufacturer's specification for equipment operation (i.e., backhoes, compactors, etc.) 10.) Employees are not equipped with adequate personal protective equipment (PPE), to include safety shoes with puncture-proof soles, when entering trash compactors and dumpsters. 11.) Hazardous chemicals/materials are not properly stored. 12.) There are unlabeled containers of hazardous chemicals/materials throughout the site. 13.) Employees are exposed to mold and biohazards resulting from leaks and sewage back-ups in the basement of the facility. 14.) Employees are exposed to trip and fall hazards in the facility due to rotten flooring and uncovered holes. 15.) The employer is not implementing precautions to protect employees from contracting and spreading the Coronavirus (COVID-19) in the workplace: a) An infectious disease preparedness and response program that complies with 16VAC25-220 has not been implemented. b) Masks, cleaning supplies and sanitizer have not been provided in that employees have to purchase and bring their own. c) A regular cleaning and disinfecting protocol has not been implemented.
