3203(a)(4): Improper cleaning of surfaces per CDC performed at this facility which performs medical treatment on Covid-19 positive patients. 3380: Four West night shift employees performing active treatment on Covid-19 positive patients are not provided with head/hair covering to protect them from droplet hazards. 5144: Employer is making Four West night employees re-use disposable N-95 respirators repeatedly. 3203(a)(4): No means to clean shoes in between rooms causing nurses to drag the virus out of the isolation rooms and into other un-contaminated areas exposing patients and employees. 3203(a)(4): Employer is forcing employees to bring in own hand sanitizing solutions to be used after exiting isolation rooms. 5144/3203(a)(4): Male nurses with beards are failing respirator fit test and are not protected while providing treatment to positive patients. 3203(a)(4): No disposable flashlights are provided in neuro unit and employees are forced to bring in own flashlights to use at work exposing patients and employees to contaminated equipment.
