1) The employer is requiring employees suspected or confirmed to have COVID-19 to work onsite, including employees who are symptomatic and awaiting test results. 2) The employer is not conducting any prescreening or surveying of employees to check for sign or symptoms of COVID-19 prior to the commencement of each work shift. 3) The employer is not requesting patients to reports signs or symptoms of respiratory illness on arrival at the health care facility. 4) Phlebotomists are required to interact with patients inside of six feet who are suspected or confirmed to have COVID-19 and not provided proper PPE to include gloves, a gown, a face shield or goggles, and a respirator. 5) When informing employees of possible exposure to other employees who have COVID-19, the employer is not keeping confidential the identity of the confirmed COVID-19 person. 6) The employer is not notifying the Virginia Department of Labor and Industry within 24 hours of the discovery of two or more positive cases of COVID-19 in the workplace.
