The employer did not communicate with employees when coworkers tested positive for COVID-19 and did not identify, evaluate, and correct the hazard from COVID-19 in the workplace by arranging for isolation and/or testing of employees who were in contact with coworkers who tested positive. The employer notified employees about one case of COVID-19, but not a second case, which resulted in the death of the employee. Family of deceased texted the employer on Aug 2 to notify them that the employee had tested positive. The employer did not communicate with the employees regarding this case until after they heard from the family of the employee that the employee had passed away.
