ER does not provide training on ATD policy to employees. ER does not have a COVID-19 response plan. OSHA investigated a fatality around April of 2020 and ER provided wrong and false information to the inspector. ER does not have any training and all of their records are fake. They asked an EE to do it but employee refused and left the company.Alleged Hazards: 2, Employees Exposed: 1
