1. Regulated waste was not stored properly. 2. Employees were required to use respirators without Respiratory Protection Program. 3. Respiratory Protection Program was not available to the affected employees. 4. Improper hazard assessment potentially exposed employees to COVID-19. 5. Improper employee trainings on respiratory protection and the usage of personal protective equipment (PPE); such as, but not limited to, COVID-19 related PPEs. 6. The employer did not have an Emergency Action Plan. 7. Personal Protective Equipment (PPE) and other controlling methods were not provided to employees who handle emergency evacuation of known COVID-19 positive patients.
