1. Multiple staff positive for COVID-19, however, no alerts have been sent to their coworkers, last year a few inaccurate attempts were made via our scheduling app, but staff only knows by sharing through word of mouth from affected staff. 2. Multiple positive patients on almost all units, again, staff not fully aware of these positives. 3. No adequate social distancing or masks being worn by all patients.
