1. The employer refuses to enforce the face mask mandate. There is a sign stating that masks are required, but it is not being enforced. 2. Management has come into work while being sick and testing positive for COVID-19, which endangered the health of employees. 3. The employer is not requiring employees to get tested for the virus and employees are reprimanded if they do get tested. 4. Several employees (at least four of twelve on staff) have tested positive for COVID-19. Precautionary measures such as enforcing face mask use, installing protective barriers between customers and employee points of contact, alerting employees of new cases and frequent cleaning have not been done.Source: Osha.gov | Receipt Date: 2021-01-12
