1. The employer is not complying with the Governor's COVID-19 mandate in regard to employees wearing face coverings. Some workers are wearing masks and fearful of those who do not. They have asked others to wear masks but upper management refuses to wear them or mandate that all employees in office wear masks. 2. The employer is not complying with the Governor's COVID-19 mandate in regard to maintaining a minimum 6-feet of separation between employees in the workplace. Employees are working within close distance (less than 6 feet apart) of each other in the workplace. 3. The employer is not complying with the Governor's COVID-19 mandate in regard to following CDC guideline for employees who test positive for COVID-19. An individual did not self-quarantine and continued to work after testing positive for COVID-19. Hazard Location: Inside the workplace. 4/14/2021 DGAlleged Hazards: 1, Employees Exposed: 1
