1. Employees are exposed to fall hazards of 15 to 20 feet when working from the mobile platform to load or close-up a trailer due to: a. There is only one guardrail on the platform and it is located on the trailer side. There is no fall protection behind the employee to keep them from falling off of the backside of the platform. b. Another employee on the ground is pushing the platform along side of the trailer. 2. Employees are exposed to struck-by and fall hazards due to the trailers are moved around when employees are still inside the them. 3. Employees are exposed to struck-by hazards due to the safeties have been removed from the pneumatic nail guns. An employee shot a nail through their foot about three weeks ago. 4. Employees are exposed to fire hazards due to: a. Fire extinguishers are at least a couple of years out of date. b. There are not enough fire extinguisher located throughout the facility. 5. Employees are exposed to COVID-19 hazards due to not the employer not requiring employee's who are exhibiting COVID-19 symptoms to quarantine or be tested. 6. Employees are exposed to ergonomic hazards due to having to single lift 100 to 150 lbs. 7. Employees are exposed to struck-by hazards due to forklifts and utility task vehicles speeding throughout the plant. 8. Employer does not allow employees to go to a doctor when injured at work. There have been three incidents over the last month where employees have been injured and medical assistance beyond basic first aid was required such as, but no limited to stitches and a prescription.Alleged Hazards: 10, Employees Exposed: 191
