1. Employees exposed to individuals having an active COVID-19 infection are susceptible to contracting a COVID infection; 2. Employees exposed to individuals having an active COVID-19 are not quarantined; 3. Employees having an active COVID-19 infection are not quarantined; 4. Physical distancing to help prevent the spread of COVID-19 to employees is not practiced; 5. Employees are not provided personal protective equipment (PPE) to help prevent the transmission of COVID-19; 6. PPE and hazard assessments have not been conducted; 7. Employees are not provided with COVID-19 screening and testing requirements; 8. A COVID-19 vaccine or vaccination series is not made available to employees; 9. Employees are not provided with medical care with regard to medical treatments related to COVID-19; 10. Appropriate cleaning and disinfection of work areas and equipment is not accomplished especially after individuals suspected of having or confirmed as having an active COVID-19 infection have been in the facility; 11. Employees are not provided with the supplies necessary for good hygiene practices to mitigate the spread of infections such as, but not limited to, COVID-19; and, 12.
