1. The facility is not regularly cleaned. Sweeping is the only type of cleaning done, and the facility is not disinfected. 2. Face masks are not required for COVID-19, although the county has a mandate for wearing the masks. 3. Monitors are held up with dictionaries and could fall and strike employees. 4. The countertops are cluttered, and there is trash and used napkins on them. 5. Computer wires on the floor are causing a trip/fall hazard. 6. The carpet in the front room has come loose from the floor and creates a trip/fall hazard. 7. In the last room, the "Cookie Cutter" room, there is a heavy magnet inside that could fall and strike employees. 8.
