1. Employer does not have COVID-19 policy in the facility. 2. Employer has employees wearing masks, but mask-wearing is not enforced. 3. Employer does not have policy and procedure for contact tracing of employees who come in contact with those contracted COVID-19. 4. Employer does not enforce 6-ft social distancing. There are more than 10 people at times, in conference rooms, and they do not wear masks correctly. 5. An employee who works at the facility, had a family member who came in contact with COVID-19 individual.
