1.) The employer is not requiring restaurant/bar staff to wear face coverings while on duty. 2.) The employer is allowing staff to work after they have tested positive for COVID-19. 3.) The employer is not informing non-infected staff when other staff members have tested positive for COVID-19. There have been (5) infected employees with-in the last 30 days at this location. 4.) The employer is not conducting contact tracing to determine if staff is being infected at work. 5.) The employer is not mandating sanitation or proper cleaning to prevent the spread of COVID-19.
