1. The employer failed to review applicable orders and guidance from the State of California and the local health department related to COVID-19 hazards and prevention. On 9/28/21, there were six or seven employees who ignored the Alameda County Mandate that all persons are to wear face masks when indoors, regardless of vaccination status. These employees were sitting within 3 feet of another in the 2nd floor conference room, which shares space with other coworkers also.
