T8 CCR § 3205(c)(3)(B)(3) - Employer sent out email on July 30, 2021 notifying employees of a COVID-19 outbreak in the workplace, but failed to notify close contact employees of their close contact until August 6, 2021. 2. T8 CCR § 3205(c)(3)(B)(4) - Employer failed to notify union representation within 24 hours of know COVID-19 cases and possible exposures. 3.
