1. Manager's Office provided to the Township Board a COVID-19 Plan in April 2020. This plan has never been provided to Department Heads or to Township Employees. 2. Daily check in log never told employees what to do until the end of July. No written direction has been provided to employees except what is written on the bottom of the check in sheet. 3. Manager would walk around the Township hall with no face mask. This person did not wear a mask until the death of a Sergeant in the police department. 4. Township employees have received no training on personal protective equipment, nor have they been notified within 24 hours of the employer being informed that an employee in the workforce had tested positive for COVID-19.Source: Osha.gov | Receipt Date: 2020-12-14
