Employees are exposed to being cut from sharp edges from plastic being cut by plastic forming machines due to lack of appropriate JHA/PPE for machine operation. Employees are exposed to struck by hazards due to reported and witnessed forklift loads that are not wrapped or balanced correctly. Employees are operated without regard to pedestrian traffic in close quarters with shelving and other objects creating struck by and caught between hazards. Employees operate forklifts without appropriate training Employees are exposed to unsanitary conditions due to no soap or towels in bathrooms Employees are exposed to slipping hazards from plastic sheeting waist and pooling lubricants and oil near machines Employees are exposed to Covid 19 hazards because employer is not utilizing JHA for Covid 19 Employees are not notified of known Covid 19 infectionsAlleged Hazards: 8, Employees Exposed: 10
