1. The employer does no not have written and implement Hazard Communication program in place, that includes sections, such as, but not limited to, training. 2. A personal protective equipment (PPE) assessment has not been performed for all job tasks and locations. 3. Employees working with hazardous chemicals, such as, but not limited to, sanitizers and disinfectants, are not provided with the proper PPE. 4. Employees have to purchase their own PPE such as, but not limited to, face coverings/mask and similar PPE, because replacements are not provided by the employer. 5. Employees are not provided a sufficient number or replacement of PPE such as mask for COVID-19.
