T8CCR 3205 - In December of 2020 employee contracted COVID-19, resulting in others being infected. Masks and social distancing were not enforced. Masks were not supplied at the time. COVID-19 positive employees were allowed to work on weekends. T8CCR 3205(c)(7)(B) - Cleaning crew not informed of COVID-19 positive cases. Cleaning was never done per CDC guidelines. Employees had to clean restrooms. T8CCR 3205(c)(3)(B) - ER failed to notify mail carrier. T8CCR 3205(c)(2)(D) - Upper management continued in person meetings after signs went up. T8CCR 3205(c)(6)(B) - New concern with new surge due to no one following protocols.Alleged Hazards: 5, Employees Exposed: 1
