1. Two staff members tested positive for COVID 19 on November 22, 2021 and the employer did not follow Pennsylvania Department of Health and Centers for Disease Control and Prevention (CDC) guidelines for informing employees of their possible exposure to COVID-19 in the workplace. 2. Employees are not wearing face coverings in accordance with Pennsylvania Department of Health and Centers for Disease Control and Prevention (CDC) guidelines for enforcing the use of face masks. 3. The workplace was not cleaned after two employees tested positive to COVID 19 in accordance with the Pennsylvania Department of Health and Centers for Disease Control Prevention (CDC) guidelines for cleaning areas after a confirmed COVID case in the facility. Hazard Location: The primary room, transition room, and intermediate room were the rooms with the most exposure and where the staff and students who tested positive are most frequently.Alleged Hazards: 3, Employees Exposed: 22
