1. A male second grade teacher regularly wears a face covering below his chin, not covering his nose or his mouth. His high school aged son is covid positive as of 5/6/2021. 2. A female fifth grade teacher wears only a face shield with no fabric enclosure. 3. A first grade teacher regularly tells students to remove their face coverings for a mask break. 4. A first grade student presented symptoms consistent with COVID-19 infection. A teacher notified the office and requested the student be seen by the FASA (school first aid assistant). The FASA was assigned to lunch room duty at the time and was unavailable to respond to the situation for over an hour, during which time the student remained in the classroom, potentially exposing the classroom teacher and other students. 5. Teachers remove face coverings in common/public areas when around other people. 6. On May 3, 2021, a confirmed case of COVID-19 was identified in a first grade classroom. Students were sent home on May 3. Some of those students were permitted to return to the school for in-person instruction on May 10, the seventh day after the students were sent home. The CCSD return to school policy for students requires the students to remain out of school for a minimum of seven days, returning no earlier than day 8. This policy was not followed.
