1. On 7/13/2021, an employee tested positive for COVID-19. The employee returned to work on 7/19/2021 still symptomatic with headache and fever. When the employee asked to go home because they were feeling ill, management denied the request. Return-to-work criteria was not met before the employee reentered the workplace, exposing employees to SARS-CoV-2, the virus that causes COVID-19. 2. Employees do not have appropriate PPE while dealing with suspected COVID-19 positive residents, including but not limited to, respirators. 3.
