An employee recently had to be hospitalized recently due to carbon monoxide exposure. 2. Employees in the facility are exposed to COVID-19. The employer does not ensure that employees follow the recommended CDC guidelines related to social distancing. Employees are required to work in various rooms within six feet of each other. 3. The facility is infested with rats. The employer has not established and implemented a continuous and effective extermination program.Alleged Hazards: 3, Employees Exposed: 20
