1. Employees are not being informed when there has been a possible exposure. 2. Employees and residents are required to drink from a 5-gallon igloo cooler that are not being cleaned and are being touched by multiple people daily. 3. Not all staff are wearing PPE while interacting with youth on medical isolation. 4. PPE was not being provided. 5. No training on donning or doffing PPE. 6. PPE worn in medical isolation units being worn back into general population units. 7.
