HR representative within the institution failed to provide employee with the material safety data sheet an no required annual notification of workplace chemical hazards. Imminent injury or death due to contaminated medical products that the University is using have not been tested for known contaminants, but are requiring these products to be used on employees creating unsafe working conditions. It is a release of bio safety material without proper safety protocols being put in place. OSHA Substance Safety Data Sheet on Ethylene Oxide - this sheet was not given to the employee none of these health risks were explained to the employee in mandatory request of being tested with this swab that contains this toxic hazard.
