New respirators were issued to all fire personnel, numbering around 70 employees, around March of 2020 without providing the following: 1. 1910.134 (c) failure of the department to implement a respiratory protection program. 2. 1910.134 (k) failure to implement proper training on new respirators specifically on how to don, check seals, and doff properly resulting in the exposure of 1 employee to a possible covid patient due to a malfunctioning mask on which he was not trained to use. 3. 1910.134 (f) failure to fit test employees after issuing new respirators. This was brought to their attention and they then provided fit testing but some employees had already been using the new respirators on medical calls. 4. 1910.134 (e) failure to provide medical evaluation or opinion after a change in working conditions would increase physiological burden on the employee (i.e., switching to a negative pressure respirator and wearing tyvek coveralls on medical calls). 5. Last physical exam by a physician has been around 12 months in time.
