1. In the weld department, employees weld on mild steel, stainless steel, and galvanized steel. Employees are not provided with adequate ventilation system or provided respirators to reduce employee exposure to air contaminates. 2. The paint/ powder coating area does not have adequate ventilation system. 3. Grinders and cutting tools are not guarded properly and/or the guards are removed. Material is being kick back at employees. 4. Grinder operators are not enforced to utilize eye protection and hearing protection is not provided. 5. Large belt grinders and sanders are not guarded properly in that guards are broken and/or removed. 6. Employees are not properly trained on press brakes and die changes resulting in exploding dies. 7. Electrical cords, extension cords, and electrical boxes are damaged and frayed throughout the facility. 8. The employer has failed to implement a COVID-19 Prevention Program to include; face covering enforcement, social distancing on the production floor and in the breakroom, contract tracing, and return to work policies. There have been at least two confirmed positive COVID-19 cases.
