1. Individuals on site have tested positive for COVID-19 and close contacts have not been informed. 2. Employees who have been in close contact with the COVID-19 positive employee have not been required to quarantine and/or obtain negative test results to return to the workplace. 3. After an individual has tested positive for COVID-19 in the workplace, proper sanitation of his/her work area has not been completed. 4. Employees are not provided with hand washing stations on the south side of the building. 5. Employees are not provided with hand sanitization devices in the facility. 6. Public water fountains are the only measure of obtaining water in the workplace and increase the risk of spreading illness. 7. Indoor mask mandates are not enforced in the workplace.Alleged Hazards: 1, Employees Exposed: 25
