1. Employee not required to wear PPE appropriately (i.e., masks not worn over the mouth and nose). 2. Employees not following proper hand hygiene protocols (i.e., servers not washing their hands or using alcohol-based hand sanitizer). 3. The employer is not following proper protocols to notify the employees when they have been exposed to COVID-19 in the workplace. No less than 10 employees have been exposed and at least five have tested positive for COVID-19 without the employer notifying the employees or closing for in depth cleaning.
