1. Management does not enforce the COVID policies or take corrective action with those who continually violate the rules. 2. Entry screening protocols are not enforced. Employees either avoid the check in procedure or lie about their condition. 3. Employees do not wear masks and the mask policy is not enforced. 4. An employee knowingly entered the facility three times while infected with COVID, or recently exposed to COVID, and was never disciplined despite being reported. 5. Most of the office, materials, engineering, IT, financing, and shipping and receiving departments are quarantined due to positive COVID tests or exposure to positive individuals. 6. Employees do not report when they have signs and symptoms or when they have been exposed to known or potential cases. 7. Exposed employees are not quarantined. 8. Incidents of workplace violence have made employees afraid to work. 9. An employee suffered a stroke on second shift and had to call for their own help as they were working alone. 10. Many injuries and illnesses have been mishandled or misreported. 11. The press room contains multiple hazards related to unsafe machinery and a lack of PPE and safety enforcement. 12.
