1. Masks wearing in the facility not consistent, supervision will speaks to large groups of employees without wearing a mask. 2. Several employees sick with COVID allowed to continue working without quarantining. 3. Not adhering 6 ft distance rule throughout most of the facility. 4. Not informing employees that other employees are sick with COVID-19. 5. The temperature machine is not accurate or functioning correctly, always registers 97.3 degrees. 6.
