1. No lockout/tag out procedure in place. 2. Guard sensors on the small work stations towards the front of the warehouse are bypassed with metal tape. 3. Employees not required to operate presses with safety glasses. (Through out the production area.) 4. Safety glasses not required in the entire building. 5. Employees towards the back corner of the production area are not required to wear face shields or safety glasses while grinding metal automotive housings. 6. Improper guards or no guards on presses. 7. Presses able to be activated with one hand. One press has levers on it that can be activated with one hand. If employee holds the levers down to long, hot hydraulic oil sprays out the hoses on top. 8. Steel toe shoes not required. 9. Improper or non-functioning blue lights on fork lifts. 10.
