1. Leaking roof on live electric wires in the building. 2. Leaking roof creates a slip hazard. An employee broke their shoulder this year because of it. 3. Safety lights on both bindery machines do not work properly. 4. The stop button on the six-pocket binder does not work. It is on pocket number one 5. Safety switches are bypassed on the Kansas inserter machine. 6. All the stackers have at least two bypassed safety switches. 7. The safety switches were removed off the S.L.S inserter sliding doors. 8. The four-pocket binder has a broken 440v plug along with many other cords in the shop. 9. Wires hanging down by the paper sheer show 'bare wire with not even tape on them. 10. Leaking high pressure air lines by the operators. 11. 440v drops not properly secured to the ceiling. If an operator pulls on the cable it may short out the 440. 12. Some of the fork trucks do not have back up horns and some of the horns do not work. 13. The emergency brake does not work on one of the fork trucks. 14. Unsanitary employee bathrooms. 15. Safety switches are tapped shut. 16. Fire extinguishers and tornado alarms are not updated nor working or properly tagged. 17. No lock out/tag out anywhere in the building. 18. Safety keys on doors and machinery are bypassed. 19. The server room has no AC and can potentially cause a fire due to boxes of paper by the hot server. 20. Full oil buckets are kept at a high temperature due to being in a closed compressor room. 21. Work tables are unstable and are jagged so they cut the operator's hands. 22. Electrical breakers are not properly marked in case of an emergency. 23. Wire wheel and grinder in maintenance does not have guards or an on/off switch. 24. No safety training for anyone. 25. Open holes in the building by the press. 26. No fork truck training. 27. The safety door on the bailing machine is bypassed where the operator loads large boxes of scrap. 28. No safety glasses are provided. 29. There are broken and hanging lights and fixtures on the east side of the building. 30. The fork trucks leak oil causing employees to fall. 31. No first aid kits on the production floor for emergencies. 32. The safety cables on the big conveyor going to the bailing machine does not work. 33. Material racks are not properly anchored. 34. Sky lift truck horn or safety lights do not work. 35. No safety harness available for sky lift truck. 36. Employee's do not know where the MSDS are kept due to lack of training. 37. No safety chains available for the lift basket for the lift truck. 38. Unmarked chemical spray bottles. 39. Eye wash stations are unsanitary and not working properly. 40. The oil separator system for the air compressor does not work causing machines to slip and is a hazard when air lines leak oil causing more oils to slip on. 41. Platforms for production workers to stand on the six-pocket bindery are not lagged in the ground so when the fork truck driver bumps the platform with a skid of material it causes the operator to fall. 42. The exhaust on the back up electric generator will leak into the shop to the point it burns your eyes, nose and causes headaches. It seems to leak more when it is warmed up. 43. 440V lines by the paper shear are not in conduit and are just hanging down. 44. The employer is not following COVID-19 precautions. 45. Employees do not wear masks. 46. Machines are not being wiped down to prevent the spread of covid-19.
