1. No COVID-19 screening, employees enter without masks, no temperatures are taken, no social distancing and masks are not provided. 2. Extension cords are stretched across floors in various locations causing trip hazards. 3. Large steel shelving units 16 to 20 feet tall with heavy steal parts on them are not anchored to the floor or to a wall. Some supports on the shelf's are bent. 4. Employees are required to dump slag dust from the Laser CNC dust collector without masks on. 5. The fork truck has a brake issue, where the brakes are always on but is driven anyway daily. 6. No safety glasses are provided. Employee had an eye injury and was told to see their own doctor and charged for a vacation day because employees are 1099. 7. Employee has suffered permanent damage to their shoulder from lifting heavy sheets of steel that are 52 x 85 inches and was told to see their own doctor.Source: Osha.gov | Receipt Date: 2020-08-24
