1. Employees and outside people are not being screened on a daily basis/there is no daily entry screening protocol for symptoms. No temperature readings or daily questions regarding COVID symptoms. 2. The employee break room is exposed to all occupants and there are no barriers when these people are coming into the facility to handle and turn in paperwork. There is no barrier between the employee's break room and workstations. 3. Non-employees are using the lunchroom and not wearing masks. 4. Employees have requested and reported not feeling well but are being written up under the attendance policy for missing work. 5. There are no dedicated entry points or physical barriers at the facility for daily screening. 6. Hand sanitizer is not being provided, kept filled and accessible at entry points and break areas around building. 7. Employees are not trained on COVID 19, including protocols for sending employees home, deep cleaning of worksite, training on PPE or any awareness of COVID 19. 8. Protocols not implemented for sharing tools, equipment, handheld scanners, forklift trucks and yard switch trucks to ensure frequent and thorough cleaning and disinfection of equipment between employees. 9. Employees not provided or required to use PPE such as rubber gloves, goggles, face shields, and face coverings for employees and contractors/drivers. 10. Proper disinfection of commonly touched surfaces such as doors to the facility, breakroom, bathroom, refrigerator, cooking appliances, time clocks, vending machines, drinking fountains, toilets, and faucets is not being done. 11. Bathroom has hand dryers that are still being used and available for use. 12. Employees have not been trained or rquired to self report to plant leaders, symptoms of COVID 19 and what to do if they're having symptoms. 13. Forklift operators are not being retrained after accidents. Two employees were involved in an accident with forklifts in March with no retraining. 14. Employees have to rush while moving freight, can't see what they're doing, and accidents are occurring.
