Safety Report: Covid-19 OSHA Complaint, Cardinal Health, 21681 Melrose Ave, SOUTHFIELD, MI, 48075, USA

3 years ago business

Regulatory Report

21681 Melrose Avenue, Southfield, 48075 Michigan, United States

Complaint mentions COVID but the main issue is access to bathroom facilities. Appears the person has been given the opportunity to use bathrooms along his route, but the Cyclotron facility (possibly not affiliated with Cardinal Health-will need to verify) is not allowing him to use the bathroom at their facility (may be part of their visitor policy to reduce COVID spread). Send regular d-letter to determine what the issues are and if the employee has access to use the bathrooms (and hand washing facilities) along the way. Include information for the ADA (1-800-514-0301) in the letter to the employee as that is the agency that needs to be informed of his concern regarding reasonable accommodations. This is a NAME REVEAL so you don't have to be completely generic, you will want to include some of the specifics so the person's issues can be adequately investigated and addressed. Send letter to the Melrose Ave, Southfield address provided in the complaint, that is affiliated with Cardinal Health. Due date: 6/9/2020 MB 6/2/2020
