1. Use of spray glues that use toluene as a solvent in unventilated room. 2. Air dust collector not rated for fumes. 3. No signage for safe tool usage and no training. 4. Safety guards have been removed from some tools. 5. No workman's compensation insurance with five full time employees. 6. Building has no windows and no ventilation in back half of building where manufacturing takes place. 7. No eye washing station. 8. No ventilation in room that uses a propane forklift. 9. Masks provided for employees but only within last couple months after employee became ill. 10. Masks are not rated properly, no new filters since original purchase.Alleged Hazards: 1, Employees Exposed: 1
