1) Employees are required to wear contaminated PPE such as N95 and disposable gowns multiple times while attending positive COVID-19 residents. 2) Employees are required to wear N95 respirators without proper fit testing or medical evaluation 3) Employer does not have staff assigned or provide housekeeping, cleaning and hygiene or sanitizing services 4) Untrained staff are assigned for COVID-19 unit 1. 5) Employer does not inform staff of positive COVID-19 cases despite multiple positive cases among staff and residents.
