1. The employer failed to report two fatalities of employees working at the Walmart Supercenter at 2500 W. 95th St., Evergreen Park, Illinois within (8) hours to OSHA as required. The employees died on March 3,2020 and March 29, 2020. Applicable OSHA Standard: 29 CFR 1904.39 2. As of March 1, 2020, the employer failed to ensure that employees exposed to COVID-19 were provided appropriate personal protection equipment (gloves, masks) and soap and/or hand sanitizer. Applicable OSHA Standards: 29 CFR 1910.132, 1910.134 and 1910.141 3. As of March 1, 2020, the employer failed to ensure the facility was cleaned and sanitized on a regular basis to prevent the spread of COVID-19. Applicable OSHA Standard: 29 CFR 1910.141
