1. The employer has not effectively developed or implemented an Infectious Disease Preparedness and Response Plan. Upon knowledge of a positive test at the workplace, the employer did not identify, informed and quarantine workers that might have been exposed. There has been two positive cases in the last week. There are sick employees, exhibiting symptoms such as fever, are still working and not quarantine or get tested. 2. The employer has not provided adequate sanitization in the facility, common areas and work areas after confirmed COVID-19 positive cases. 3. On 12-08-2020, there was a fire on the second shift at the facility. Employees evacuated but returned to work without the facility being properly ventilated exposing them to smoke inhalation hazards.Alleged Hazards: 3, Employees Exposed: 200
