I am requesting an on-site inspection to look into the hazards located at Choice Financial Group, LLC dab Choice Insurance Agency. On January 10th and 11th, 2021 the employer did not follow the manufacturers manual/specifications for operating the Ozone Generators, in that, 1. The Ozone Generators are being run for up to 72 hours or more in the workplace. 2. After the Ozone Generators are shut off, the workplace spaces are not adequately ventilated prior to being occupied by workers. 3. The Ozone Generators cause papers to curl-up, desk discoloration, and leaves a sticky substance on surfaces. 4. Employees are not informed when Ozone Generators are going to be used in the workplace. 5. Employees are experiencing medical episodes from exposure to Ozone.Alleged Hazards: 5, Employees Exposed: 1
