'- Employees came back to work after two weeks recovering from COVID are not wearing masks properly. - Aides are not wearing masks in patients rooms. - Not practicing social distance. - No cleaning services with knowledge on COVID. - No fire training. - No emergency Evacuation training. - No In-Service for nurses, CNA, Med-Techs, and aides. - No In-Service for housekeeping to include COVID. - Employees are provided N95 with stains. - There are not enough N95's for the employees. - No training on COVID to include CDC guidelines to follow in the Nursing Care Facility.Alleged Hazards: 12, Employees Exposed: 20
