1. Employees are not physically distanced six feet or more as required in the CDC and Stay Safe MN Guidance, regardless of whether or not face coverings, gowns and booties are worn. 2. Employees who have had close contact with employees who were diagnosed with COVID-19 have not always been informed of the close contact. 3. Employees who have been identified as close contacts of employees who were diagnosed with COVID-19 have not been kept out of the workplace (asked to quarantine), regardless of whether or not face coverings, gowns and booties are worn. 4.
